Individual Grant Request
Form:
If you would like to request assistance from City Youth Now on behalf
of an individual youth, please download this form and return it
to us by mail or fax.
Requests must be submitted by a probation
officer, social worker, CASA, attorney, or other adult authorized
by the court to provide services to youth. (Sorry, but we cannot
accept requests directly from youth or their caregivers.)
2.
The child for whom you are requesting
assistance must be under the jurisdiction of the San Francisco Juvenile
Court system or recently emancipated.
3.
City Youth Now cannot provide funds for
reimbursements and we must make checks payable directly to the vendor.
4.
Please allow 2-3 weeks for a response.
(If this is an emergency request, please indicate this on the form
and call the office to alert us about the request.)
